Are you running 6.5MR, and not a more recent overlay like 6.5.5m? If so,
I think you are tripping over a bug... upgrading to 6.5.5 should eliminate
the gfx pipe crashes.

Of course, it could be a HW problem as Allan points out (always good to run
irsaudit to eliminate possible HW problems). But, this one actually looks
like a bug which we fixed.
