X Error and PF Notice

Has anyone had any experience with errors like the following?

GL: Warning: failed to create context
X Error of failed request: BadAlloc (insufficient resources for
operation)
  Major opcode of failed request: 145 (GLX)
  Minor opcode of failed request: 5 (X_GLXMakeCurrent)
  Serial number of failed request: 0
  Current serial number in output stream: 28
PF Notice: Caught SIGCHLD. Exiting due to death of
child with pid 12540.
CAVE tracker exiting (Process 12545)

For those of you who will know what I'm talking about...
I'm trying to start up a CAVERNSoft / LIMBO application on an Onyx
10000, running a 4-wall CAVE (4th is floor). The OS is Irix 6.5.2.
I've run the pthread_fix already and that didn't clear it up. It dies
right after CAVERNInit and never starts the app. This doesn't happen on
our Octanes running the simulator. In fact, it doesn't always happen in
the CAVE, but I want to know what circumstances tend to cause it.
