It looks like you're not the only one experiencing this problem...
another customer has reported the same problem using different graphics on
a w2k machine. (Just curious, which flavor of NT are you using?)
At this point I'm unable to reproduce the problem so it's a little
difficult to debug but at first glance it sure seems like the problem has
something to do with graphics cards that support dual head configs. Still,
I've been unable to reproduce the problem here.
Can you try compiling and running simple.c to see if this works?
If it does then I'll dig into input.c once again... I also have a
separate test program (based on complex.c) that I can send you to test if
you're interested in helping us figure out what the underlying problem is.
