We too have had problems with dangling semaphores after closing pfCAVE
applications. We also experienced a program lockup with CAVELib 3.01 that,
according to VRCO, was semaphore related. You can see if you have the same
problem by running cavevars (which is an OpenGL CAVE diagnostic app) for
longer than 2-3 minutes. For us, after around 2 minutes, cavevars would
freeze. VRCO has a beta release of CAVELib that will avoid this problem.

We are running Performer 2.5, IRIX 6.5.11m, on an Onyx2. I've also seen
the same thing on an Octane 2 running IRIX 6.5.14m. We have been cleaning
them up (the semaphores) with the rmsem script that Allan mentioned.
