I have used Multigen to reduce faces by hand. Not fun but, doable.
Had to reduce a CAD model to something I could show in realtime.
Multigen allowed me to selectively retain vertices than I needed
to retain the basic shape (texture put back the details).
I've had similar experiences with polygon reduction tools. I would
suggest trying one to see what you get then either start over or
clean it up by hand.
Clearly, designing the model for realtime up front is a better idea.

In my experience most of the polygon reduction tools end up
distorting the model when you start getting to the kind of
reduction levels your talking about! So depending on your
model you may be better off rebuilding.
One of our guys just finished doing this for a client. The database
he worked with was building used for a walk through! A lot of the
work was replacing parts of the model with lower polygonal objects.
He used Multigen because of it's compatibility with performer.
My wife uses Maya to model ocean going vessels then exports them
to inventor using alias. Since they are used in a simulator, she
has to be conscious of the polygon count and builds them accordingly!
