No space left on device on xfs filesystem with 1.6TB free

We've seen this on 64-bit 2.6.23.x and 2.6.27.x kernels with inode64 
mounted partitions. xfsprogs 2.8.11 (Debian Etch).

Unfortunately we've never been able to track this down.
