If you'd like to provide an xfs_metadump image of the fs, that might be a first
step to trying to reproduce it here.

Can provide it on a side-channel if you'd rather not attach it to the bug. 
With that, your xfsdump invocation, and some machine details, we might be able
to reproduce it and make more progress.
