Sorry for not having anyone look at your bug for so long time.  These issues are
serious and mean your filesystem got corrupted.  One typical case for it might
be that you ran with the write cache enabled, but without barriers which don't
work over device mapper.  Make sure you have your disk write caches turned off
with such a setup.

And yes, xfs_repair needs to be run when this happens.
