KDB 4.1 USB keyboard fix

Hi,

	Some Itanium 2 platforms don't have PS/2 port. So, the only way to connect a keyboard to these machine is via a USB port. How can we use the KDB patch without USB support in this case?

I am removing USB keyboard support from kdb in v4.2.  The USB code for
2.5 kernels is useless, I have neither the hardware nor the time to
support USB keyboard for kdb and nobody has volunteered to keep the kdb
USB code up to date.
