kdb setup/breaking into kdb?

kdb is entered after a failure.  kdb can be entered manually using the
pause key on a PC keyboard or control-A on a serial console, as long as
a program is reading from the serial console.  To enter kdb during boot

append="console=tty0 kdb=early"
