This doesn't seem like the (now infamous) 'dir2 bug'
(http://oss.sgi.com/projects/xfs/faq.html#dir2) but to be certain as you ran
2.6.17 at some point can you verify this problems persists with a clean/sane
filesystem please?

(ie. use a recent kernel and xfsprogs and make sure all is happy, see the above
URL for details)
