I re-did the whole thing, minus the LVM patch, and it's working great
now.

PS: You guys may want to change the documentation that's on your web
site. In the FAQ, it addresses the issue of XFS on LVM, and recomends
applying the LVM patch. Since it obviously causes breakage (at least for
the recent kernel) maybe you should indicate that. I might save someone
else some hassle.

Yes, definitely. The XFS tree is at beta6, and we've done QA on that.
I wouldn't recommend beta7 for production use.
