Document how pmlogger access controls effect pmlc behaviour.

pmlogger - improve pmlc access controls
Specifically, don't execute pmlc's "new volume" nor "sync"
commands unless the pmlogger access control allows mandatory
operations.
Also clean up the semantics of the control variable used to
hold access capabilities, and add comments around the parts
of the code that are subject to access control restrictions.

pmlc/actions.c - improved error handling
With tightened access control enforcement at pmlogger, some
PDU exchenges can now return a error PDU in places that were
not being handled correctly in the pmlc code.
