Updates to the programmers guide introducing pmdammv/libpcp_mmv
Chapter 4 is now more generic topic, covering application instrumentation
using pcp libraries in general. Initial discussion of MMV PMDA and API
is added, new images are used to explain how MMV works is introduced.
