Abstract common code from MMV for dealing with dynamic names.
Added pmdaTreePMID, pmdaTreeName, pmdaTreeChildren, and also
pmdaTreeRebuildHash (see comments in pmda.h) which implement
the dynamic namespace handling iff the PMDA author chooses
to use the __pmnsTree and __pmnsNode lookup mechanism (using
code from libpcp also).
