    Further updates to the programmers guide
    
    Up to Chapter 4 now reviewed.  Changes include:
    - completed updates to interface docs which now have thread-safe
    variants (pm*Str_r)
    - update references to tmpname, long-since replaced by mkstemp
    - additional event type metric references in appropriate places,
    including type table for pmExtractValue and so on.
    - consistent code formatting throughout, removing the occasional
    odd whitespace

    Further updates to the programmers guide
    
    Up to section 3.8.5 (Timezone Services).  Changes include:
    - correct path parameterisation of pmcd.conf and pmcd.options
    - several updates related to the namespace api routines
    - platform independent (not IRIX) examples, demos, references
    - add initial words describing event metric type, add pointer
    to a new section on this (wip)
    - begin updates to interfaces docs which now have thread-safe
    variants (pmErrStr_r)

    Updates to the programmers guide through the end of PMDA install.
    
    Moves us onto start of Chapter 3.  Changes in this round include:
    - updates to code snippets for trivial and simple PMDAs, and the
    associated text, to match numerous recent advances - running both
    DSO/daemon for trivial, running as non-root, IPv6 options, helper
    methods for progname.
    - update PMDA debugging section to match how its usually done and
    introduce use of LOG_DEBUG rather than straight printf.
    - document the Install forced_restart variable
    - document the python/perl PMDA Install variables
    - platform independence in a few more spots using explicit .so
    - update Configuring Tools section to mention pmlogconf alongside
    existing pmieconf words.
