pcp updates

Ken has attempted to use pcp on Solaris and promptly discovered a
bunch of deficiencies which I've tried to address. While working
on the stuff he needed I've discovered a few other minor problems
in the Solaris PMDA which I've also attempted to plug.

The following changes since commit 3300d8ecf365ba7a7365a3177f1de730fcd66300:
