Document pmcd user and group access control specifications
Extend the host-based access control description with details
of the user/group model as well. Also add missing text about
the PMCD_SOCKET environment variable.

Add man page for pmdaAttribute callback, related man updates
New man page for the attribute callback in version six and later
of the pmdaInterface structure. Rework parts of pmdaMain to be
firstly more uptodate, and secondly more complete (esp. re the
client termination callback).
