After recent PDU changes to support per context state in PMDAs, need
to rework these low-level test programs and scripts that are manipulating
PDU headers directly.

assorted - pmcd PMDA using INTERFACE_5 and cputime metrics
Changes in QA to track recent changes in the pmcd PMDA, mostly
the addition of pmcd.cputime.* metrics and moving to
PMDA_INTERFACE_5.
