[fam] [prepatch] Directory Notification

Hi.  I've implemented directory change notification for linux 2.3.99pre8.
I've done some light testing, but more eyes are always welcome.  I'm not
sure that doing the notifications from within vfs_* is the right thing
to do; could the filesystem stacking guys take a look?  This includes
some of the work from Manfred on making kill_fasync SMP-safe.

It's unrelated to the imon work done by SGI but serves the same purpose,
hence their inclusion.
