Changes since 1.6: +51 -56 lines

Turn xfs_bmbt_{get,set}_{all,startoff,startblock,blockcount} macros into
real routines because they're so expensive.  Add asserts for them as well.

Clean up the STARTBLOCK macros a little, for 32 vs. 64 bits.
