In several places we directly convert from the XFS inode
to the linux (VFS) inode by a simple deference of ip->i_vnode.
We should not do this - a helper function should be used to
extract the VFS inode from the XFS inode.

Introduce the function VFS_I() to extract the VFS inode
from the XFS inode. The name was chosen to match XFS_I() which
is used to extract the XFS inode from the VFS inode.

Version 2:
o don't use vn_to_inode() and inode_to_vn() functions as they
  are not needed

vnode cleanup in xfs_fs_subr.c

Cleanup the unneeded intermediate vnode step in the flushing helpers
and go directly from the xfs_inode to the struct address_space.

kill the v_flag member in struct bhv_vnode

All flags previous handled at the vnode level are not in the xfs_inode
where we already have a flags mechanisms and free bits for flags
previously in the vnode.

decontaminate vnode operations from behavior details

All vnode ops now take struct xfs_inode pointers and the behaviour
related glue is split out into methods of it's own.  This required
fixing xfs_create/mkdir/symlink to not mess with the inode pointer
but rather use a separate boolean for error handling.  Thanks to
Dave Chinner for that fix.

propogate return codes from flush routines

This patch handles error return values in fs_flush_pages and fs_flushinval_pages.
It changes the prototype of fs_flushinval_pages so we can propogate the errors
and handle them at higher layers.  I also modified xfs_itruncate_start so that
it could propogate the error further.

Revert broken and obsolete change to page invalidation.

The changes checked in under mod xfs-linux-melb:xfs-kern:27909a
are broken. 2.6.20 will release with the troublesome warning
turned off so by reverting this mod we will have the same
behaviour as 2.6.19 w.r.t to fs_tosspages and fs_flushinval_pages.

Unmap pages before removing them from the page cache.

The new cancel_dirty_pages() code found that XFS was removing
page from the page cache that had dirty page table entries.
XFS invalidates page cache pages via internal interfaces which
are implemented via truncate_inode_pages which does not remove
the page mapping first.

Switch to using invalidate_inode_pages2_range() which does almost
the same thing except it also removes page table mappings as
expected by cancel_dirty_pages.

Start writeout earlier (on last close) in the case where we have a truncate down followed by delayed allocation (buffered writes) - worst case scenario for the notorious NULL files problem.  This reduces the window where we are exposed to that problem significantly.

There is a slight change in the license terms and conditions words
to go with the copyrights, so most of the files are not getting
new GPL's, just updated versions ... but there are 20-30 more files
here as well.
