vfile_alloc no longer takes a vnode/vsocket pointer to setup.  Instead the setup is done by vfile_ready.  It's ok to defer it in this way because the open file is unusable until vfile_ready.  The benefit is that now only vfile_ready ever sets the vnode/vsocket that is associated with an open file.

change all #if _K64U64 => #if _MIPS_SIM == _ABI64 and equivalent.
The original meaning/intent of the _K64* and _K32* stuff has been getting more
and more ignored, and now it is just confusing.  #if _K32U32 and equivalent
is just gone, we have no machines like that since 5.3.  #if _K32U64||_K64U64
have the ifdefs removed, and code remaining, since it is always true.
#if _K32U64||_K32U32 usually changed to != _ABI64.  Some files have the
older longer _MIPS_SIM_ABI64 and similar shorted to _ABI64 and equivalent;
the compiler folks tell me this is preferred now and in the future, and
is what cc -show prints, so it is less confusing.

variable used to hold the open vnode needs to be different from
the one used to hold the return vnode. This needs to be enforced so
that the locking on the open vnode interposition chain doesn't get messed up
when the return vnode is different from the original opened vnode.

Fix tlbmiss panic from copying an entire fid struct.  Not all of it
is actually allocated, and when one ends up at the end of a page if
the next page isn't valid that's bad.  Fix is to only copy the used
portion of a fid.
