When called from  mk_incore_fstree, the startblock parameter is in increasing order.
This means the new insert has to be at the end of the applicable bcnt extent list.
The current implementation traverses the applicable bcnt extent list for each insert.
The more the inserts, the longer it takes.
Added last field to avl struct.

1) Eliminate compiler warning (COMP)
2) Bug fixes (BUG)
3) Reduce CPU cycles. (CPU)
1) Define 64 bit quantities to prcess bmap bits in chunks of 16.
   XR_E_UNKNOWN_LL, XR_E_FREE1_LL, etc. (CPU).
   
2) Make search_dup_extent an inline function

3) Make findfirst_inode_rec and find_inode_rec as inline functions

4) Make is_inode_reached, add_inode_ref, drop_inode_ref, is_inode_referenced and 
   and num_inode_references as inline functions.
