Up to [Development] / failsafe / FailSafe-mgr

Added GPL/LGPL copyright headers & documentation for open source release, and removed CXFS stuff

This is sysadm_failsafe, a Rhino-based GUI for setting up & administering
FailSafe high-availability clusters.  (It's referred to in many places as
fsmgr, short for FailSafe Manager.)

For installation instructions, see the INSTALL file.  If you're a fsmgr
maintainer, see README.maint for additional information on generating
Makefiles and such.

For the terms under which the FailSafe GUI is licensed, see the COPYING
file.  (To summarize: mostly LGPL, some GPL.)
