On you FAQ you were asking if this could be repeated, and I have a case,
100% reliable way of repeating this on my server.

Polaris is a HP Netserver LH3 with a Highpoint 4-channel IDE controller
running software raid 5 over 4 channels. Its root disk is a pair of 9.1
gig scsi drives as a hardware raid using the onboard AMI Megaraid
controller.

uname -a output:
Linux polaris 2.6.8 #2 SMP Sat Oct 9 16:04:48 BST 2004 i686 GNU/Linux

The fault is 100% reliable after 1.5 hours doing a considerable dump of
many 2-6 meg files over a samba connection. It does not matter if this
is a server push or pull.
This is the sysylog output:
