So - you're testing this with CONFIG_LBD enabled I assume; does it fail on small devices (several gigabytes for example)? You can use -dsize=10g for example to restrict mkfs.xfs to making a smaller filesystem. There are known device driver issues with multi-terabyte MD devices in current kernels, and I suppose the same sorts of issues may still be lurking in other drivers too.
