the 'over and over again' part is just due to how xfs_repair works... it
unlinks lost+found then rediscovers all those 'lost' inodes.  rename it
out of the way to make those 'errors' go away.

see what "file /bin/bash" in the chroot says, maybe - or hexdump it, see if
it's nulled somehow.  looks like it's corrupted one way or another.  probably
the same goes for init... if bash & init are in bad shape, it doesn't bode
well for the other files.

did you see any device i/o errors in your logs?
