Today I have got this bug!!! On clear host:
I did:
Power on
login
> sync
> nice ls -R /
> cp /etc/X11/Xmodmap /tmp/Xmodmap
> vi /tmp/Xmodmap
change one strings
:w!
donn't quit
POWER OFF
result:

First time I got this bug about 7 years before in Irix 5.3 XFS on my mail box
(in /var/mail/...). It still exists in Irix 6.5.x.
