It turnds out that the kernel that I was testing with had the experimental
Direct I/O for NFS patches applied (but disabled) as part of a NFS client
fix/patch bundle.

The patch changes the interface to the direct IO operations; as XFS was not
in-tree, it was not modified.  Fixing this manually (or backing out the
offending patch) resolves the problem.
