What kernel are you using:
2.4.19 stock plus 2.4.19 XFS full patchset from SGI website
Running rh7.3, XFS volume is on a Dell Powervault with LSI Logic MegaRAID 1.57
Description of Problem:
I ran XFS 1.0 and 1.1 without any problems for 18 months.
After upgrading to rh7.3 and 2.4.19, within 24 hrs I got this:

I have no idea how to replicate this, and I'm not sure I want to ;)
