I don't know about a 3ware controllers but I'd say the smart on the disk on port 6 is telling the controller that it's going bad.
Better check. xfs_repair might be able to fix your filesystem.

What 3ware controller are you using?

Any idea how to fix the problem.

We do have
Linux hermes-clued0 2.4.17-0.4XFSsmp #1 SMP Tue Mar 12 14:18:33 CST 2002 i686 unknown
patched kernel with XFS.

Can you upgrade to something more recent please. You definitely want the XFS 1.1 release or later. Use CVS or the split patches if you want newer then 2.4.18.
