The XFS 1.0.1 release with 2.4.5. A little bit old. Intel HW.

Better update to something more recent like the 1.1 release. It fixes a lot of issues including the file corruption during power-off. I do remember that someone else encountered this with a read_only opened file as well but not what the cause was.

Upgrading to 1.1 gives you a lot more robust XFS kernel then what yuo have now and it's handling of open files will possibly be very different in behaviour then what you see now.

It might just be your lucky day, if you only knew.
