Eric,
I had the same problem with the installer, that rescue mode did not work,
I found that the problem is in the modules included in the boot disk, some
whare in the anaconda-xfs.patch you have a line in the file
anaconda/scripts/mk-images.i386 which loads the vfat file system modules.
here you should also load msdos and fat along with vfat.

I just created a hybrid installer using the latest xfs userspace tools and
the redhat updates over the 1.0.1, but since I hacked away many of the
install screens my patch would need to be regenerated ( no sgi confirm
screen, no firewall config, many extra packages in comps etc etc)

As Seth mentions In case I dont have any non xfs or ext2 which are in the
kernel the rescue mode works fine but when I have vfat / fat / ntfs
partitions too it hangs
