None of the default config files had entries for XFS or KDB so they
defaulted to off or caused problems for unattended installs. This
patch updates all defconfigs to append:

CONFIG_PAGE_BUF=y
CONFIG_XFS_FS=y
CONFIG_XFS_DMAPI=y
arch/{i386,ia64}/defconfig also have these options appended:
CONFIG_KDB=y
CONFIG_KDB_OFF=n
CONFIG_FRAME_POINTER=n
