It turns out we now have a way around the BH_End_io flag, and can run
without it, we can also use fewer special purpose end_io functions and
more of the generic cases.

        - Fixup buffer head flag printing code.

linux/fs/pagebuf/page_buf.c - 1.69
        - Cleanup buffer flag setting

linux/fs/pagebuf/page_buf_io.c - 1.71
        - Cleanup end_io handling for buffer heads, we no longer need as many
          special purpose handlers, or the BH_end_io flag.

linux/drivers/md/raid5.c - 1.12
        - Remove BH_End_io checks - the flag is gone
