XFS was not leaving highmem pages kmapped after the prepare_write
call. This reworks when we map and unmap pages in pagebuf to give
the expected behavior. Probably will not affect people using xfs
except maybe with loop devices on highmem systems.

- Do the correct thing with prepare_write and commit_write and kmapping
pages.
