Pick up changes from Steve for handling flush on delalloc.
Delalloc is still turned off by default.
However, PLEASE TURN IT ON for testing purposes.
This will enable us to find bugs before
the switch from delwri -> delalloc is thrown
for good.

- Fix flush code to convert delalloc extents. Attempt at handling
unwritten extents, also under delalloc. Use STATIC for better
debugging.
