TAKE - fix the read/modify write behavior of extending a file

Don't read buffer heads which are beyond eof from disk. Speeds up
small sequential writes big time.

        - Fix the prepare_write function so that it does not read buffers
          beyond the end of file from disk. This fixes the AIM regression.
