One more thing to try for a reproducer if the above doesn't work.
mkfs, populate, sync as shown in the original comment.
unmount
make an xfs_metadump image of the device
xfs_mdrestore back to the device
mount the device with your mount options
do the "> testfile" test
does that reproduce it? if so, please provide the xfs_metadump image.
