As Eric and Dave said your first trace defintively shows I/O errors from the
raid controller. The second one shows traces in the allocator code.
There have been various issues with high NFS loads in earlier kernels, but they
didn't show as corruption in the allocation btrees.
Either way 2.6.18 is extremly old, and without a recent kernel like
2.6.27.something I don't think we can debug more.
Also please make sure to run xfs_repair over the filesystem as the RAID
controller error could have caused various forms of filesystem corruption.
