Hm, one more tidbit. With -w 4096 1K blocks still fail:

==== BLOCKSIZE 512 ====
==== BLOCKSIZE 1024 ====
FAILED
==== BLOCKSIZE 2048 ====
==== BLOCKSIZE 4096 ====
==== BLOCKSIZE 8192 ====
==== BLOCKSIZE 16384 ====
==== BLOCKSIZE 32768 ====
==== BLOCKSIZE 65536 ====

With -w 8192, 16k blocks fail:

==== BLOCKSIZE 512 ====
==== BLOCKSIZE 1024 ====
==== BLOCKSIZE 2048 ====
==== BLOCKSIZE 4096 ====
==== BLOCKSIZE 8192 ====
==== BLOCKSIZE 16384 ====
FAILED
==== BLOCKSIZE 32768 ====
==== BLOCKSIZE 65536 ====

With -w 16384 many fail:

==== BLOCKSIZE 512 ====
==== BLOCKSIZE 1024 ====
FAILED
==== BLOCKSIZE 2048 ====
FAILED
==== BLOCKSIZE 4096 ====
FAILED
==== BLOCKSIZE 8192 ====
FAILED
==== BLOCKSIZE 16384 ====
FAILED
==== BLOCKSIZE 32768 ====
==== BLOCKSIZE 65536 ====

and similar for larger -w values...

With -t 512, 1024, 2048, and 4986 block sizes fail.
With -t 1024, only 2048 and 4096 block sizes fail.
With -t 2048, all block sizes pass.
With -t 4096, 8192 block size fails.
With -t 8192 or larger, all block sizes pass.
