I'm very tempted to chalk this up to an md problem; the error you encountered
says that xfs read a buffer which did not contain the magic that it expected. 
When this comes immediately after a disk loss and raid-reshuffle from md, it's
hard not to wonder if md did not handle the loss well, and gave xfs some bad 
data?
