pmrep: improve metrics collection via extend_indom
Switch pmrep to always use extend_indom. This gives us:
* much shorter time to start up
* ability to report new instances when appropriate (archive/zabbix)
* allows to adjust CSV output a bit, no quotes around empty values
* few unrelated for-loop cleanups as a bonus
Resolves Red Hat BZ #1377464.
