Small books consistency tweaks and improvements
Make Programmers Guide man-page-reference section refer to
PCPIntro (consistently with UAG), & tweak words around the
SEE ALSO section.
After chatting to Stan re pmdumptext, noticed pmchart had
no entry in the monitor/vis tools section - add one in.
