I've updated this document to version 1.1.   Check the revision bars for
sections that have been added/changed/deleted from the previous version
1.0 of the document.

The hacking, pruning and retiring of old version support is almost done
in the pcp4 branch of my pcp tree.

Still interested in feedback and input.
