Remove hard-coded setting of layout margins.
These were inserted by the original version of Qt designer we
used and may be contributing to the menu/font oddity (maybe).

Fix PMCD view so it works without a hard-coded path to pmcd.
Ditto for PMDAs, and including paths with Windows separators.

Fix recording of metrics with an instance domain
Currently the code incorrectly always uses the 0th
instance of each metric with an indom.
