2.1.11 bug

   Hello!

   Apache pmda, added in 2.1.11 to standard distribution, uses installed
includes, but it is automatically compiled. So PCP does not compile on fresh box
where the includes are not yet installed.

   Solution is to rename (or remove, if you don't need it) the src/pmdas/apache
directory, do make install and then (rename back and) compile the pmda.
