Lockmeter patches now available for 2.2.13 and 2.3.28

The Lockmeter patches are now available against 2.2.13 and 2.3.28.
I made a few small changes that move some of the i386-dependent stuff out
of arch/i386/kernel/lockmeter.c and into various header files.  This doesn't
make Lockmeter architecture-independent, but it does go a small way toward
helping others to port the code to non-Intel platforms.
